Procedure for Food Facility Registration

 

1. Down a registration form at from C-TECH web site from here:

      a. If you have a newer version of Adobe Reader: 

      BF you have an older version of Adobe Reader :

                               Form 3537

 2. Download a SERVICE AGREEMENTAdobe (PDF)

3. Or e-mail TO mail@fdaus.net or send a fax to (1)-319-466-9122 to request for a registration form and a contract agreement to be sent to you by fax or E-mail.

4. Completed the registration form and sign the agreement. Send the two to C-TECH by one of the following methods:

5. Wait for C-TECH to send you your “FDA food facility registration number”
 
6. Send first year’s agent fee to C-TECH by ways detailed in “Fees” Section.
